Before the COVID pandemic, we hit the national target of 95 per cent of one-year-olds fully vaccinated. Our child vaccination rates were among the best in the world.<\/p>\n\n\n\n<p>Vaccination protects children from potentially severe illnesses such as measles, mumps and whooping cough. These diseases can cause severe pain, put children in hospital, risk their lives and leave them with ongoing health problems.<\/p>\n\n\n\n<p>But Australia\u2019s vaccine success is quickly <a href=\"https:\/\/www.health.gov.au\/resources\/publications\/2025-phn-childhood-immunisation-coverage-data?language=en\">slipping away<\/a>. After the pandemic, the share of one-year-olds who are fully vaccinated kept falling. In some areas, it\u2019s now barely 80 per cent.<\/p>\n\n\n\n<p>The risks are real. Whooping cough notifications are the <a href=\"https:\/\/ncirs.org.au\/whooping-cough-cases-are-their-highest-level-35-years-so-why-surge\">highest<\/a> since records began, 35 years ago. In America, dozens of people have been <a href=\"https:\/\/www.cdc.gov\/measles\/data-research\/index.html\">hospitalised<\/a> with measles already this year, and Canada has <a href=\"https:\/\/theconversation.com\/canada-loses-its-official-measles-free-status-and-the-us-will-follow-soon-as-vaccination-rates-fall-269090\">lost<\/a> its measles elimination status. An <a href=\"https:\/\/www.bbc.com\/news\/articles\/c99jyexve1jo\">outbreak<\/a> in London is putting children in hospital, and may force unvaccinated children to stay home from school.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Why aim high?<\/h2>\n\n\n\n<p>One-year-old fully immunised babies have received vaccinations for diphtheria, tetanus, pertussis (whooping cough), polio, hepatitis B, <em>Haemophilus influenzae<\/em> type b (Hib) and pneumococcal disease.<\/p>\n\n\n\n<p>High vaccination coverage is necessary to achieve herd immunity: the point where diseases find it hard to spread to children who aren\u2019t vaccinated. Some children aren\u2019t vaccinated because they are too young. Others can\u2019t be vaccinated because they have weakened immune systems.<\/p>\n\n\n\n<p>When 95 per cent of children are vaccinated, it\u2019s difficult for even highly infectious diseases such as measles to spread in the community, protecting both the vaccinated and unvaccinated.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Sliding backwards<\/h2>\n\n\n\n<p><a href=\"https:\/\/grattan.edu.au\/news\/new-data-on-australias-childhood-vaccination-rates\">Grattan Institute analysis<\/a> shows that over the past five years, Australia has recorded an unprecedented slide in the proportion of one-year-olds who are fully vaccinated. In the year to 30 September 2025, 92 per cent of one-year-olds were fully vaccinated, compared with 95 per cent in 2020.<\/p>\n\n\n\n<p>Many parts of Australia are now well below national vaccination targets. Five years ago, 56 per cent of regions and suburbs were achieving the national target for one-year old vaccination. Today it is just 18 per cent.<\/p>\n\n\n\n<figure class=\"wp-block-image aligncenter\"><a href=\"https:\/\/images.theconversation.com\/files\/718718\/original\/file-20260217-66-uffnqs.png?ixlib=rb-4.1.0&amp;q=45&amp;auto=format&amp;w=1000&amp;fit=clip\"><img decoding=\"async\" src=\"https:\/\/images.theconversation.com\/files\/718718\/original\/file-20260217-66-uffnqs.png?ixlib=rb-4.1.0&amp;q=45&amp;auto=format&amp;w=754&amp;fit=clip\" alt=\"\"\/><\/a><figcaption class=\"wp-element-caption\">Notes: SA3s are geographical areas typically covering a population of between 30,000 and 130,000 people. Grattan Institute analysis of Department of Health, Disability, and Ageing (2026)<\/figcaption><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\">Some areas are falling further behind<\/h2>\n\n\n\n<p>The declines have been biggest where children were already more vulnerable.<\/p>\n\n\n\n<p>In the 10% of areas with the highest vaccination for one-year olds, uptake slid by just 1.3 percentage points since 2020 \u2013 from an average of 98 per cent in 2020 to 97 per cent in 2025.<\/p>\n\n\n\n<p>But in the areas with the lowest vaccination, the fall was more than four times greater, at 5.7 percentage points \u2013 from an average of 90 per cent in 2020 to 84 per cent in 2025.<\/p>\n\n\n\n<figure class=\"wp-block-image\"><a href=\"https:\/\/images.theconversation.com\/files\/718719\/original\/file-20260217-56-9k89qq.png?ixlib=rb-4.1.0&amp;q=45&amp;auto=format&amp;w=1000&amp;fit=clip\"><img decoding=\"async\" src=\"https:\/\/images.theconversation.com\/files\/718719\/original\/file-20260217-56-9k89qq.png?ixlib=rb-4.1.0&amp;q=45&amp;auto=format&amp;w=754&amp;fit=clip\" alt=\"Chart showing that vaccination rates have fallen most rapidly in areas that were already lagging behind\"\/><\/a><figcaption class=\"wp-element-caption\">Notes: SA3s are geographical areas typically covering a population of between 30,000 and 130,000 people. And every state has areas with sharp falls.<\/p>\n\n\n\n<p>Some of the biggest surges in the share of one-year olds who are fully vaccinated are in:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Bankstown, Sydney, from 92.2% to 84.8%<\/li>\n\n\n\n<li>Keilor, Melbourne, from 95.8% to 88.8%<\/li>\n\n\n\n<li>Gascoyne, Western Australia, from 95.6% to 76.9%<\/li>\n\n\n\n<li>Nerang, Queensland, from 94.1% to 82.2%<\/li>\n\n\n\n<li>Barkly, Northern Territory, from 96.2% to 87.0%<\/li>\n\n\n\n<li>Meander Valley and West Tamar, Tasmania, from 92.6% to 83.5%.<\/li>\n<\/ul>\n\n\n\n<p>There is no single profile for communities with dangerously low vaccination. Grattan Institute analysis of Department of Health, Disability, and Ageing (2026), Department of Health and Aged Care (2023), and ABS (2025)<\/figcaption><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\">Why the decline?<\/h2>\n\n\n\n<p>It has become much harder to get children vaccinated, and it\u2019s not down to a single factor.<\/p>\n\n\n\n<p>Instead, a major <a href=\"https:\/\/ncirs.org.au\/sites\/default\/files\/2025-10\/National%20Vaccination%20Insights%20project%20report%202025.pdf\">survey<\/a> suggests a mix of psychological barriers to acceptance and practical barriers to access.<\/p>\n\n\n\n<p>Misinformation and the intense debate around COVID vaccines has likely eroded trust in childhood vaccination. Among parents with unvaccinated children, <a href=\"https:\/\/ncirs.org.au\/sites\/default\/files\/2025-10\/National%20Vaccination%20Insights%20project%20report%202025.pdf\">almost half<\/a> don\u2019t think vaccines are safe.<\/p>\n\n\n\n<p>But practical barriers matter too. <a href=\"https:\/\/ncirs.org.au\/sites\/default\/files\/2025-10\/National%20Vaccination%20Insights%20project%20report%202025.pdf\">One in four parents<\/a> whose children are only partially vaccinated say it\u2019s difficult to get an appointment when their child\u2019s vaccination is due.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Governments have a plan \u2013 now they need to act<\/h2>\n\n\n\n<p>Australia\u2019s federal and state governments must tackle both types of problem.<\/p>\n\n\n\n<p>They agree. They are gearing up to respond to this emerging public health crisis with a new <a href=\"https:\/\/www.cdc.gov.au\/resources\/publications\/national-immunisation-strategy-australia-2025-2030\">national vaccination strategy<\/a>, agreed last year. It sets the right directions by emphasising building trust in vaccines, strengthening the immunisation workforce, using data to target effort and increasing accountability for getting results.<\/p>\n\n\n\n<p>But the true test will be federal and state government budgets released in coming months. Those budgets must make new investments that turn the strategy into decisive action.<\/p>\n\n\n\n<p>The investments should span the full gamut of the strategy, including:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>public advertising<\/li>\n\n\n\n<li>combating misinformation by better understanding community beliefs, tailoring government information and advertising, and helping health workers engage effectively with sceptical patients<\/li>\n\n\n\n<li>modernising data systems to track trends and focus effort<\/li>\n\n\n\n<li>delivering vaccination more often in more places, such as workplaces, community centres, and homes.<\/li>\n<\/ul>\n\n\n\n<p>Crucially, tougher targets are needed to stop some communities falling behind, and funding for local efforts, tailored to local needs, to help them catch up.<\/p>\n\n\n\n<p>Australia has hit ambitious vaccination targets before. Getting back to pre-pandemic levels will be harder than achieving them the first time, so governments must step up and redouble their efforts to protect Australia\u2019s children.<\/p>\n\n\n\n<p><em>This article is republished from <a href=\"https:\/\/theconversation.com\">The Conversation<\/a> under a Creative Commons license.
